Norton Medical Group recognizes the amount of time and effort required to complete the application and licensing process for any new position. This section provides step-by-step guidance. Should you have questions or need assistance at any step, contact our recruitment team.
All new providers who wish to be employed by Norton Medical Group must complete the following:
- Complete application through Norton Human Resource job listing
- Application for Kentucky and/or Indiana medical licensure (See below for detailed instructions.)
- Completed Norton Managed Care Credentialing application through ECHO link sent via our physician provider services once position is accepted
- Apply for DEA certificate via U.S. Drug Enforcement Administration Application
State medical licensure process
To apply for a medical license in Kentucky and/or Indiana, follow the steps listed below.
Allow at least 90 days for processing. Request receipts and keep all documentation of payments for reimbursement purposes upon your start date.
Step 1: Go to the Federation of State Medical Boards website. Set up an account with the Federation Credentials Verification Service (FCVS) by filling out the online application and all other necessary information. You will need to create a username and password.
Kentucky requires physicians to complete the FCVS process before applying for a Kentucky medical license.
Once you have supplied all the information, the FCVS will complete screenings, verifications and processing and send the information to the state medical licensure board(s) for Kentucky and/or Indiana. You can choose additional designations at any time, but there will be an additional charge for each one. Norton Healthcare will reimburse the fee for Indiana licensure only if you are required to apply for a medical license in both Kentucky and Indiana.
After your application is completed and submitted, print a copy of your receipt.
Step 2: Gather the following documents:
- Photocopy of your medical diploma
- Certificate of completion for all residency and fellowship programs, if applicable
- Your original certified birth certificate or original certified passport; the Federation of State Medical Boards will return these documents to you after verification is completed
- Photocopy of your medical school transcripts
- All other necessary FCVS forms

Step 3: Apply for a medical license for Kentucky and/or Indiana.
For Kentucky: Complete the application for state licensure with the Kentucky Board of Medical Licensure. The licensing fee is $300. Request a temporary license as well. The Kentucky Board of Medical Licensure designates coordinators, assigned based on the first initial of the applicant’s last name, to assist in addressing any questions or concerns. For Indiana: For information about Indiana licensing, go to Indiana Professional Licensing Agency website. To apply for an Indiana medical license, go to “Instructions and Application for MD/DO License” and fill out the application. The fee is $250. Note: Norton Healthcare will reimburse the fee for Indiana licensure only if you are required to apply for a medical license in both Kentucky and Indiana.
